Lamictal Side Effects: What to Know About Lamotrigine

Lamictal (lamotrigine) is a mood stabilizer widely prescribed for bipolar disorder, particularly for preventing depressive episodes in bipolar II. It has become a first-line treatment because of its relatively favorable side effect profile compared to other mood stabilizers and its unique effectiveness against bipolar depression. Understanding Lamictal's side effects helps you know what to expect and what to watch for during treatment.

Common Side Effects

Most Lamictal side effects are mild and tend to diminish over the first few weeks of treatment. These include headache (the most frequently reported side effect, usually manageable with over-the-counter pain relievers), dizziness or lightheadedness, nausea, blurred or double vision, drowsiness or fatigue, insomnia (depending on timing of dose), tremor, and coordination difficulties. Unlike many other psychiatric medications, lamotrigine is weight-neutral — it typically does not cause the significant weight gain associated with lithium, valproate, or atypical antipsychotics. It also has minimal impact on sexual function compared to SSRIs, making it better tolerated for many patients.

The Rash Warning: Stevens-Johnson Syndrome

The most serious risk associated with Lamictal is Stevens-Johnson Syndrome (SJS), a rare but potentially life-threatening skin reaction. This is why lamotrigine MUST be started at a low dose and increased very slowly (typically over 6-8 weeks to reach therapeutic levels). This slow titration is not optional — it is a critical safety measure. Any rash that develops during Lamictal treatment should be reported to your psychiatrist immediately. While most rashes that occur during lamotrigine treatment are benign, the medication is typically discontinued as a precaution until the rash is evaluated, because SJS cannot be reliably distinguished from benign rashes in its early stages. Risk factors for serious rash include rapid dose escalation, concurrent use of valproate (which doubles lamotrigine blood levels), and younger age.

Drug Interactions

Lamotrigine's blood levels are significantly affected by other medications. Valproic acid (Depakote) approximately doubles lamotrigine levels, requiring a slower titration and lower target dose. Carbamazepine, phenytoin, and phenobarbital decrease lamotrigine levels. Hormonal contraceptives (estrogen-containing) decrease lamotrigine levels, which can cause breakthrough symptoms — and levels increase when the pill is stopped during the placebo week, creating a cycling effect. Your psychiatrist must account for these interactions when dosing.

Benefits That Make It Worth Considering

Despite the rash risk, lamotrigine remains one of the most prescribed mood stabilizers because it is the most effective mood stabilizer for preventing bipolar depressive episodes (which are the most disabling phase for most patients), it is weight-neutral, it has minimal cognitive side effects ("brain fog" is less common than with other mood stabilizers), it does not require blood level monitoring (unlike lithium and valproate), and it is generally well tolerated long-term. A thorough psychiatric evaluation helps determine whether Lamictal is the right choice for your specific mood disorder presentation.
