
Major depressive disorder (MDD), also called clinical depression, is one of the most common and disabling psychiatric conditions worldwide. MDD goes far beyond ordinary sadness or temporary discouragement. It is a clinical condition characterized by persistent changes in mood, thinking, behavior, and physical functioning that last at least two weeks and represent a clear departure from a person's normal baseline. Without treatment, episodes of major depression can last six months or longer and tend to recur — with each untreated episode increasing the likelihood of future episodes.
To receive a diagnosis of MDD, an individual must experience at least five of the following symptoms during the same two-week period, with at least one being depressed mood or loss of interest: persistent depressed mood most of the day nearly every day, markedly diminished interest or pleasure in activities (anhedonia), significant weight change or appetite disturbance, insomnia or hypersomnia, psychomotor agitation or retardation, fatigue or loss of energy, feelings of worthlessness or excessive guilt, difficulty concentrating or making decisions, and recurrent thoughts of death or suicidal ideation.
These symptoms must cause clinically significant distress or impairment in social, occupational, or other important areas of functioning. The presentation of MDD varies considerably between individuals — some experience predominantly sad mood, while others present with irritability, numbness, or primarily physical symptoms like fatigue and pain. Recognizing the signs of depression in all their forms is essential for timely diagnosis.
MDD is not a single uniform condition. Clinical specifiers help psychiatrists tailor treatment to specific presentations. Melancholic depression features profound loss of pleasure, early morning awakening, and worsening mood in the morning. Atypical depression involves mood reactivity (mood brightens in response to positive events), increased appetite, hypersomnia, and heaviness in the limbs. Postpartum depression occurs during pregnancy or within four weeks of delivery.
When depressive episodes alternate with periods of mania or hypomania, the diagnosis shifts to bipolar disorder, which requires a fundamentally different treatment approach. This distinction is one of the most important reasons to seek evaluation from a psychiatrist rather than relying solely on primary care for depression treatment — misdiagnosing bipolar depression as unipolar MDD and prescribing antidepressants alone can trigger manic episodes.
MDD treatment typically involves psychotherapy, medication, or a combination of both. Cognitive behavioral therapy and interpersonal therapy have the strongest evidence base for depression psychotherapy. For moderate to severe MDD, combining therapy with antidepressant medication produces better outcomes than either treatment alone.
First-line antidepressant options include SSRIs like sertraline (Zoloft) and SNRIs. Bupropion (Wellbutrin) is often chosen when fatigue and low motivation are prominent or when sexual side effects from SSRIs are a concern. For treatment-resistant depression, options include medication augmentation strategies, ketamine-based treatments, and TMS therapy.
